### Remote Patient Monitoring with Mountain Community Health,

Managing chronic conditions such as hypertension, COPD, or diabetes can be challenging, but our Remote Patient Monitoring program is here to help. This program is designed to detect changes in your health early, provide timely interventions, and ensure you receive the highest quality care.

**How It Works:**

1. **Initial Contact**  
   RemetricHealth will connect with you to explain the program and determine if it’s the right fit for your needs.
2. **Home Monitoring Device**  
   You’ll receive a medical device, such as a blood pressure monitor or glucometer, delivered directly to your home.
3. **Meet Your Nurse**  
   Your dedicated nurse will guide you through setting up and using your device. They’ll also monitor your readings daily, offering personalized support to help you manage your condition effectively.
