How It Works:

1. Initial Contact
   RemetricHealth will connect with you to explain the program and determine if it’s the right fit for your needs.

2. Home Monitoring Device
   You’ll receive a medical device, such as a blood pressure monitor or glucometer, delivered directly to your home.

3. Meet Your Nurse
   Your dedicated nurse will guide you through setting up and using your device. They’ll also monitor your readings daily, offering personalized support to help you manage your condition effectively.
